This California Privacy Rights notice supplements the Privacy Policy for Sequel and applies to California residents under the California Consumer Privacy Act, as amended by the California Privacy Rights Act.
Information We Collect
Depending on your interactions with Sequel, personal information may include identifiers and contact details, account information, order and transaction records, communications with support, and website or device activity. We collect information directly from you and through the services described in our Privacy Policy.
When you request telehealth services, information may also include medical history, health information, and information needed for provider review and care.
Your Rights Under California Law
Subject to applicable exceptions, you may request access to the categories and specific pieces of personal information we hold about you, deletion of personal information, and correction of inaccurate information. You may also exercise applicable rights to opt out of sale or sharing and limit use or disclosure of sensitive personal information.
We do not sell your personal information. Our Privacy Policy describes how information is used and shared. We will not discriminate against you for exercising privacy rights.
How to Exercise Your Rights
Submit a verifiable request through hello@joinsequel.com. We may need information to verify your identity and authority to act, including when an authorized agent submits a request. We respond within applicable time limits and will notify you when an extension is permitted and needed.
Some information may be retained where necessary to complete transactions, provide requested services, protect security, or comply with legal obligations.
Health Information
Certain health information may be subject to HIPAA or other laws and exemptions. See our Privacy Policy for information about protected health information.
